A Consent to Treat form must be completed for all participants, coaches and manager.

Only the Current Consent toTreat will be accepted. Electronic Signatures are not valid; form must be sign in ink by the parent or guardian.

 

 Note: The valid current consent to treat is dated 2/16. Nothing else is accepetable
